Lifecycle and status gates

By default an investigation moves through these statuses. Your organization can rename them or add others.

Status Meaning
Calibration Scoping review that decides whether to investigate.
Input Evidence and interviews are being gathered.
RCA Root cause analysis is under way.
Management Review Findings and corrective actions are with management.
Closed The investigation is complete.
Declined Declined at review. A justification is required.

On the case page, open the status menu (Move to…) and pick a status. You can move to any status, forwards or back; there’s no fixed order. To close a case, move it to Closed. To reopen it, move it back to an earlier status. A closed case can still be edited.

You can also set the Assignee, chosen from the team and the case’s creator.

A status gate is a list of requirements a case must meet before it can enter a status. Gates are checked on the way in, whichever status the case comes from. Admins set them up in Investigation settings.

If a case doesn’t meet a gate, the move is refused and you see Requirements not met, with each missing requirement and a link to fix it, for example “The case has 0 evidence files; it needs at least 1” with Open the evidence.

A gate can require:

Requirement The case must have…
Field filled in A value in a chosen field.
Team size At least this many team members.
Team role filled A team member holding a chosen role, such as Sponsor.
Meetings logged At least this many meetings, optionally of one type.
Linked events At least this many concern reports. The source record counts as one.
Evidence uploaded At least this many evidence files.
Risk matrix complete A value in every cell of the risk assessment matrix.
RCA complete At least one causal factor, each classified with a root cause, and every root cause with a description, details and a corrective action.
Corrective actions decided No corrective action still waiting for a management-review decision.
Action items complete Every action item created from an accepted corrective action complete.

To decline an investigation, move it to Declined. You’re asked to record why in Decline this investigation. The justification is always required, even if no gate is set up, and stays on the record. If the case is reopened later, it’s kept and shown as Decline Justification (previously declined).
