Investigation settings

Set up how investigations work for your organization in Settings → Investigations. There are five tabs: Team Roles, Methodologies, Field Labels, Risk Matrix and Status Gates.

You need to be an admin with write access to Investigations to change these settings. See Investigations for how they’re used on a case.

The roles people can hold on an investigation team. Roles are labels: they don’t change who can open a case.

  • Add Role: enter a Name, for example Scribe.
  • Rename: everyone holding the role keeps it under the new name.
  • Delete: only when nobody on any team holds it. Change their role first.
  • Use the arrows to reorder.

Sponsor and Facilitator are Built-in: you can rename them but not delete them.

A methodology is a root-cause map your organization analyzes cases against. The list shows Safety Radar’s published templates (marked System) and your organization’s own maps.

  • New Methodology builds a map from nothing.
  • Create from this (on a template) or Duplicate (on one of yours) starts from a copy.
  • Set as default chooses the methodology new cases start on. It’s marked Default.
  • Rename and Delete manage your own maps.

Rename investigation fields to match your organization’s language. Leave a field empty to keep its default name. The fields are Title, Narrative, Investigation Driver, Methodology, Status, Location, Location Note, Source Record, Source Record ID, Management Sponsor, Facilitator, Proposed Completion Date, Decline Justification and Report. Select Save Changes.

The grid every investigation is scored on. Nothing is set up by default.

  1. Under Consequence rows, add a row for each type of consequence, for example Asset & Property Damage, and select Add row. Rename, reorder or remove rows as needed.
  2. Under Metric columns, choose a metric and select Add column. Each column uses that metric’s scale of values. Only metrics with a scale of distinct values can be used.

Metrics themselves are defined in Settings → Metrics.

What a case must have before it can enter each status. For how gates behave on a case and the full list of requirement types, see Lifecycle and status gates.

  1. Find the status you want to gate. A status without requirements shows Open gate.
  2. Select Add requirement, choose the type, and set its options, such as a count (At least), a role or a meeting type.
  3. Add as many requirements as you need, then select Save Changes.

The statuses themselves (names, order, colors) come from your organization’s status flow.
